Perkins signs as Lieutenant Colonel of the 19th Regiment. Signed by Captain Frederick K. Fletcher, 2nd Lieutenant D. B. Holmes, and 1st Lieutenant Tyler Stearns. Includes signatures (or marks) of and information regarding payment to soldiers of Company F between October and December 1864. States that the Company was paid $3,950.67 on 24 March 1865.
