Includes comments, time of enlistment, and place of appointment. On verso, contains "Proof of Effectives" chart, noting officers present and absent. Lieutenants Eli Elmor and David Moore sign, certifying the authenticity of the chart. Colonel Richard Humpton, commander of the Second Brigade of General Benjamin Lincoln's Division, signs 3 September 1777.
